Health Inclusion Nursing Team

The Health Inclusion Nursing Team consists of Registered General Nurses, Mental Health Nurses, Associate Practitioners and administration staff working in communities across Dundee City, covering all localities.

Holistic health assessments
Health consultations
Mental health nurse assessments and support
Identify and support unmet health needs
Promote and support engagement
Deliver nurse led drop in clinics
Close partnership working with statutory and voluntary services
Person-centred goal oriented work

Who we support

City wide targeted approach
Homelessness and those at risk of homelessness - with dedicated health and homeless nursing team
Community justice and prison leavers - with co location in community justice
Individuals affected by substance and alcohol use
Carers
Humanitarian outreach work with refugees and asylum seekers
Ethnic groups
Individuals and groups who are hard to reach or find it difficult to engage
People living in areas of deprivation and/or poverty
